Animal Hospital Wiki (fan) — Anomaly Guide master list, based on the Fandom breakdown; game is patched almost daily, the total drifts with updates.

How to Spot Anomalies — Core Principles

  • Check layer by layer. Many anomalies only reveal themselves on CCTV or in photos — the window alone is not enough.
  • Use the stand-behind trick for "Staring at the Player": a real patient will not eerily track you.
  • Refresh CCTV multiple times. Unnatural Body re-randomizes its distortion each time the CCTV reopens.
  • Compare photos line by line — eyes, ears, horns, fur color against the patient.
  • One confirmed red flag = reject. Hesitation is often more fatal than a misjudgment.

Decoys: These Are NOT Anomalies

Slime on the floor, fires in rooms and similar environmental scares are decoys/events — they do not mean the patient at the window is an anomaly. Rejecting real patients because of them is a classic beginner mistake. Judge only by confirmed tells on the patient itself.

Frequently Asked Questions

How many anomalies are in Animal Hospital?
The community usually says about 14, but the Fandom breakdown lists 17 individual clues across four detection layers (Visual, Photo, CCTV, Documents). The game is patched almost daily, so the total drifts with updates.
Which anomaly is the easiest to identify?
Three Eyes — three glowing red eyes is the most cited "must be an anomaly" sign in the community. Once spotted, you can generally reject immediately.
Is a fire in a room or slime on the floor a sign of an anomaly?
No. Environmental scares like floor slime and room fires are decoys/events, not patient flaws. Judge anomalies only by confirmed tells on the patient — one confirmed red flag means reject.
